For four decades, Hospice & Community Care has been dedicated to serving our community with Hope, Comfort, and Compassion. As we celebrate our 40th anniversary, we invite you to join us for a special Memorial Day Service on Thursday, May 22, 2025, at 9:00 AM in the Veteran Garden.

During this tribute, we will honor the Veteran patients we have been privileged to serve , recognize all Veterans cared for by our agency, and hold a blessing ceremony for the newly placed Tour of Duty Stones—a lasting tribute to their service and sacrifice.

A Meaningful Keepsake
To mark this milestone, we are offering a custom 40th-anniversary dog tag as a special way to honor your loved one’s legacy. Each tag will feature our anniversary logo on one side and your loved one’s name on the reverse side. Tags are available for $20, with all proceeds benefiting our Veterans Programs.

How to Participate
We encourage families to bring a photo of their loved one to display next to their Tour of Duty Stone during the ceremony. If you would like to attend or purchase a commemorative dog tag, please use the below form to register by May 8th!
