About Hospice & Community Care
Hospice & Community Care is a not-for-profit health care organization founded in 1985 to provide physical, emotional, social and spiritual care for terminally ill patients and their families. Hospice & Community Care is licensed and is a certified Medicare/Medicaid provider.
Mission
The mission of Hospice & Community Care is to give hope, comfort, and compassion to our community when needed most.
History
Hospice & Community Care began with one York County woman’s struggle with cancer and her desire to remain at home until her death. The mutual efforts of her family social worker and pastor led to the formation of the first local hospice agency which was incorporated in 1983 and began providing care in 1985. This was a new option for end of life care in a community that had previously relied on institutional medical treatment for the dying.

Service Area
Hospice & Community Care serves residents of a six-county area which includes York, Chester, Lancaster Cherokee, Fairfield and Union counties in South Carolina.
