A Team Approach to Care

Hospice & Community Care provides services of care through professional medical staff which includes physicians, nurses, C.N.A.s and home health aides. Social workers and spiritual and bereavement counselors complete the team approach to care. Following are the members of our healthcare team.

Physicians
Physicians play an essential role in the care of Hospice patients. Patients and their families have the ability to choose to have their physician follow them or use our Hospice physicians to develop and oversee the patient plan of care. Physicians are in regular communication with the Hospice team to assure that the patient’s medical needs are taken care of during their time with Hospice

Nurses
Our registered nurses are experts in pain control and management of physical symptoms that cause distress or interfere with patient comfort. Nurses visit the patient on a regular basis and watch for changes that might require attention. They also play a key role in assuring that the patient is living in maximum comfort and with dignity. Nurses work in partnership with our medical director and the patient's physician to make sure the medical plan is up to date. A nurse is on call 24 hours a day, seven days a week, to answer caregiver questions. Nurses also make emergency visits.

Home Health Aides
Our home health aides play a vital and much-appreciated non-medical role. They make regular visits to provide personal services to the patient such as help with bathing, dressing and personal care. They may also assist with certain housekeeping chores to help keep patients independent and comfortable. They offer good humor and kindness and are highly valued by families and caregivers.

Social Workers
Our social workers help families with questions about insurance coverage, financial issues or other end-of-life concerns. They know about community resources and make referrals to other agencies when appropriate. They are skilled at listening carefully, then finding solutions to social and emotional issues that arise at the end of life.

Grief Counselors
Grief counseling and a broad range of grief services are available to patients and the families. Counseling is also available to any member of our community who has experienced a loss. Our counseling show people how to cope with their grief and begin the journey of personal healing. We also provide individualized programs on loss and grief to churches, schools, community groups, businesses, nursing homes and other organizations.

Chaplains
Our chaplains provide non-denominational and non-judgmental spiritual counseling, if requested, and grief support. They respect all faiths and traditions and have worked with patients of all beliefs systems. Chaplains understand that the end of life, while a cause for sadness, offers opportunities for joy, family reconciliation and celebration.

Volunteers
Volunteers are at the heart of what we do. Volunteers spend valuable time with patients and caregivers, bringing strong hands and a special sensitivity to each family and each situation. Their contributions are endless. One of their most valuable roles is to sit and talk with patients, and they often form strong lasting bonds with the families they serve. Volunteers drive caregivers to the store. They help with household duties. They write letters, make phone calls, care for pets. In many cases volunteers have helped patients achieve a final wish.
