10 Things to Know About Hospice & Community Care

1 As the fifth hospice established in S.C., and the largest in the area, we serve patients
in York, Chester, Lancaster, Cherokee, Fairfield and Union Counties as the major non-profit hospice since 1985.

2 Hospice & Community Care provides quality services of care, compassion and support. We provide emotional, spiritual and medical care for patients at the end of life, support for families and caregivers and programs to educate the community.

3 Hospice care is appropriate when the primary goal of care becomes comfort and relief of symptoms, rather than a cure for terminal illness. For a patient to enter hospice, a physician must certify life expectancy of six months or less “if the disease follows its normal course.”

4 50% of the patients we serve have a non-cancer diagnosis.

5 Services provided by Hospice & Community Care go to anyone who needs them regardless of ability to pay. Out-of-pocket costs are usually a very small amount for issues related to the terminal illness.

6 Medicare Hospice Benefit provides for medications, durable equipment, registered nurse support, home health aide visits and social work visits for patients who are eligible for Medicare.

7 Our staff of nurses, with over 150 years of experience, is on-call 24-hours-a-day and make visits on holidays and weekends when needed.

8 Two full-time doctors on staff make house calls and assist the patient’s physician in taking care of pain and symptom management issues.

9 Hospice & Community Care’s services can be given in a variety of settings such as nursing homes and assisted living facilities, as well as the patient’s home.

10 As one of our most important assets,
over 300 volunteers guided by certified professionals provide a substantial portion of the care we provide our patients and their families.
