Transitions can help.
How does Transitions help?
Transitions is a free community program for people coping with a life-limiting illness of 12 months or less.
Transitions provides support through compassionate volunteers and a knowledgeable Transitions Coordinator who work together with you and your family to find the resources you need.
Transitions Coordinator
The Transitions Coordinator will do an initial assessment of your needs, and will then link you with community resources. The Coordinator will also make regular phone calls and home visits, as needed, to ensure you and your family are receiving all you need.
Transitions Volunteers
Transitions Volunteers offer friendship, a listening ear, help with errands or light housework, and even a break for tired caregivers. Volunteers are specially trained and willing to lend a helping hand when needed.
Although Transitions does not provide medical care, you do need your physician’s approval to join the program. The Transitions Coordinator will maintain contact with your physician to ensure you are receiving continuity in your overall care.
